NYC Department health LogoThe Clinical Systems Improvement program supports quality improvement initiatives across primary health care sites in New York City, focusing on the delivery of preventive services and the management of chronic diseases. 

IANet worked with several clinics to customize and implement a computer-based health registry on diabetes management.  We took over the project management of this initiative and were asked to confirm accuracy of the registry reports, enhance its user interfaces, streamline maintenance, and provide onsite assistance to users working with the tool.

  • Customized the computer-based registry for use in this project
  • Built a robust graphical display of the clinics’ progress trends for patient care
  • Installed the application at several community-based health centers and provided training to them on how to maximize the health registry for plan care with their patients.
