Visiting Nurse Service of New York, NY
With over 9,860 highly skilled care providers, VNSNY is the largest not-for-profit home health care agency in the nation. In 2005 they made 2,200,000 professional home visits to more than 115,000 patients. Their caregivers travel throughout New York City and Nassau and Westchester Counties, seeing an average of 30,000 patients each day.
VNSNY was the lead partner on a national collaborative project to reduce the number of acute hospitalizations (ReACH) within home care agencies. IANet was asked to create a website that would support the project’s collaborative activities and develop a measurement system that makes it easy to submit and view the participants’ progress. IANet created a website that allowed its users to upload resource materials, share their progress and experiences, submit their data, and view reports on that data in table and chart formats displaying the trends.
- Created a secure website with searchable knowledgebase, archived discussions and participant profiles. Includes hierarchical measurement that provides details to individual agencies, their sponsoring Quality Improvement Organization and national reports to guide the project’s faculty in managing the project.
- Based upon a role-based system, the website provides multiple cohorts to simultaneously use cohort specific resources on the site.
